There is an identification number on the Official Business Jury Questionnaire which is located above your name.  To access the on-line questionnaire, you’ll need to enter that ID# in the space provided below on this page.  (See example*).   Once you are logged in, the number identified as “Barcode Number”  will be a six-digit number and will be different than the ID# on the document you received.
